PAIN-TO-WIN is a top-down horror shooter set inside a hostile asylum functioning as a meat grinder: a closed, conspiratorial environment shaped by death cults, paranoia, and fragmented hierarchies. You play as an organ extractor, earning money by bounty hunting madmen, psychos, and other violent degenerates. The game draws from ugly, uncomfortable PSX-era maniac aesthetics through handcrafted, highly detailed non-pixel art visuals.
A system built on violence, organ trade, and routine brutality. Inmates, dealers, and contractors operate as parts of the same mechanism.
Fragmented non-linear world structure. Wandering, detours, dead ends. Some paths loop back. Others lead nowhere. Nothing guides forward.
Hostile combat. The environment degrades alongside the subject. Rooms fill with debris, blood, and bodies. Clarity is not guaranteed.
Interactions with NPCs: inmates, cultists, dealers, and other human waste. Talk, trade, ignore, or kill.
Each weapon has its own way to harm.
Perks and upgrades come from extraction, trade, and brain surgery.
